(a) (1) An operator who wishes to register and obtain a license as required by § 816.02 shall apply to the Code Enforcement Officer or his or her designee.
(2) An application form may be secured from the office of the Code Enforcement Officer or his or her designee and shall set forth the following information:
A. The name and business address of the operator;
B. A plan of the premises showing the location where the juke box, mechanical or electronic amusement device, pool table or other similar machine or device is to be displayed or maintained, as well as the location of the supervision of the establishment;
C. The type, manufacturer and serial number of each juke box, mechanical or electronic amusement device, pool table or other similar machine or device for which the application is being made; and
D. Such other information as the Code Enforcement Officer or his or her designee may deem necessary for the proper administration and enforcement of this chapter.
(b) Upon approval of the application, the Code Enforcement Officer or his or her designee shall issue a license for each location where a juke box, mechanical or electronic amusement device, pool table or other similar machine or device is to be displayed or maintained, and may also issue a seal to be affixed to each juke box, mechanical or electronic device, pool table or other similar machine or device registered and licensed under this chapter.
