(a) A seller shall provide a purchaser of a dog with a health record for the dog at the time of sale.
(b) The health record shall include:
(1) Information on the breed, sex and age of the dog, if known. If the dog is advertised or represented as being registered, the name and address of the pedigree registry organization where the dam and sire are registered shall be indicated;
(2) A record of the vaccinations and veterinary treatment received by the dog while in the possession of the seller;
(3) The name and address of the seller along with a statement affirming that all of the above information is accurate to the best of the seller’s information and belief; and
(4) Signatures of both the seller and the purchaser.
