157.05 MILITARY SERVICE-CONNECTED INJURY; DEATH AND RETIREMENT BENEFITS.
   (a)   Military Service - Connected Injury Benefits.
      (1)   For necessary absence from duty due to any military service-connected injury, as distinguished from illness, full-time employees shall be compensated at their regular rate, for not more than a total of twelve weeks during the employee’s tenure within the City.
      (2)   The City Manager may, at any time, require proof satisfactory to him that the injury is military service-connected and that the benefits herein provided should continue.
   (b)   Death and Retirement Benefits.
      (1)   When a full-time employee of the City retires through length of service, or upon death, he shall be paid unused sick leave in accordance with bargaining agreements, or policy set forth for non-collective bargaining employees, with the minimum standards as outlined in Ohio R.C. 124.39. When an employee of the City participates in the sick-time buy-back program, he shall be paid up to a maximum of 960 hours.
      (2)   Each full-time employee may designate his beneficiary and may at any time change his beneficiary. The City Auditor shall maintain a current City employee beneficiary file.
         The file shall contain the “Designation of Beneficiary” forms which shall contain the following information: the name, address and marital status of the employee naming a beneficiary; the name and address of his spouse; the names and addresses of his children; the name of the beneficiary designated by him together with a statement that he desires the beneficiary named to receive any unused sick leave pay accumulated by him; and the employee's full payroll signature with the date of execution.
      (3)   If no beneficiary is designated or if the named beneficiary is deceased at the time provided for payment, the unused sick leave shall be paid to the surviving spouse of the employee; if none, to the children of the employee; and if none, the unused sick leave pay shall revert to the City.
