Application for a permit to construct, open, operate and maintain a private range, or for a gun club to use a private range, shall be filed with the City Manager in a form prescribed and furnished by him. Such application shall state:
(a) The name of the applicant;
(b) Kind of permit requested;
(c) Name and address of range;
(d) Names and addresses of officers and members of the club;
(e) Type of firearms to be used;
(f) The type of target to be used; and
(g) Such other information as the City Manager may require.
