Any applicant for a permit shall complete the application form designated for the purpose by the Director of Public Works and in addition shall provide the following.
(A) An approved 8-l/2 inch by 11-inch drawing of work to be performed.
(B) Location of where the work is to be done.
(C) Name and address of the person, public utility, or firm completing the work.
(D) Duration of the work to be done and whether all necessary permits have been obtained from other city, state, and federal agencies.
(E) Date the work is scheduled to begin and projected date of completion.
