§ 31.52 PROCESS OF ESTABLISHING A COVERED ACCOUNT.
   (A)   As a precondition to opening a covered account in the city, each applicant shall provide the city with personal identifying information of the customer (valid photo identification card, address, phone number, date of birth, employer and if renting and deemed necessary by the customer service representative a copy of the lease. For accounts being opened over the phone, if the customer service representative suspects a problem, then it will be required that the customer fax a copy of the drivers license and lease if renting). Such information shall be entered directly into the city's computer system and shall be filed in a secure location.
   (B)   Each account shall be assigned an account number which shall be unique to that account.
