§ 50.093 APPLICATION FOR PERMIT PREREQUISITE TO CONSTRUCTION.
   (A)   No connection shall be made with sewerage system until the owner of the premises, or his duly authorized agent, shall have made application to the city in the manner specified in this subchapter, and the application shall be made and a written permit granted before any portion of the drainage system of the house or other connections shall be laid or constructed. The application shall be made on a blank form provided for that purpose by the city, and shall state the name of the applicant, the location of the premises, the name of the licensed person by whom the work is to be done, the size of the house drain, house sewer, location of branch fitting in house sewer, and the entrance of main house drain where actual house connection is applied for. Significant users will require a detailed application as required in the city's sewer use and pretreatment ordinance.
   (B)   The application shall contain a specific agreement to obey and abide by any and all resolutions, ordinances, rules and regulations that are now, or may hereafter be passed by the City Council for the protection of the sewerage system and to restrict, regulate and control the use thereof and the connections therewith. The application shall also give permission to the Director of Public Works and other officers of the city to enter the premises of the applicant at any reasonable time, to look after or attend to anything connected with the sewerage system or the plumbing connected therewith. The application shall be filled out in ink and signed by the applicant or his or her duly authorized agent.
