§ 94.03 DISPLAY PERMITS.
   Any not-for-profit group of three or more persons may obtain a fireworks display permit for consumer fireworks by complying with the following procedures:
   (A)   At least one member of the group shall be a resident of the village;
   (B)   All members of the group shall be 18 years of age or older; and
   (C)   At least three members of the group shall sign an application form provided by the village which will contain the following information, as a minimum:
      (1)   The name, address and telephone number of the members of the group applying for the license;
      (2)   The date, time and location of the fireworks display for which a permit is sought; and
      (3)   The name, address and telephone number of the person who will supervise the fireworks display, together with evidence that the person is licensed by the state as a lead pyrotechnic operator.
