Each application for a permit to operate a motorized golf cart/UTV on the village roadways shall include:
(A) The name and address of the applicant;
(B) Other information as the Village Board of Trustees may from time to time require;
(C) The Village Inspector shall conduct an inspection of the golf cart/UTV prior to issuance of the permit to assure that the golf cart/UTV has all required equipment; and
(D) In addition, each permit application, whether for an initial permit or a renewal, shall be accompanied by evidence of a liability insurance policy.
