§ 116.02  LICENSE REQUIRED.
   (A)   (1)   No person shall engage in the business of hauling trash, garbage, rubbish, refuse, ashes or debris over or along any of the streets or alleys of the town without first having applied for and obtained a license to do so from the Clerk-Treasurer’s office.
      (2)   The fee for such license shall be $25 payable at the time of the filing of the application.
      (3)   The license shall be for a one-year period commencing on the date that the license is issued.
   (B)   In applying for a license, the applicant shall indicate the following:
      (1)   The frequency of said pickup;
      (2)   The number of people working on the truck;
      (3)   The location and address of the disposal of the trash, garbage, rubbish and refuse;
      (4)   The type and size of vehicle used in the operation;
      (5)   Name, address, telephone number, Social Security number and age of the applicant; and
      (6)   Types and amounts of insurance coverage for damage to property and for death and/or injuries to persons.
