Beginning six months after a franchisee first begins to provide cable service in the County, a franchisee shall complete for each quarter within 30 days after the end of the quarter a report containing the following information:
(1) a summary of customer service requests identifying the number and nature of the requests and their disposition;
(2) a summary showing the number, address, and nature of service calls during the quarter;
(3) the total number of subscribers at the end of the quarter;
(4) details of any special offers made to subscribers that concluded during the quarter or lasted for more than 90 days as of the end of the quarter;
(5) results of any proof of performance tests conducted on the cable system during the quarter; and
(6) the status of any rebuild, any deployment of facilities affecting the cable system, or any major new construction in progress during the quarter.
