Eligibility to be a participant in a plan is determined by the provisions of the plan. On becoming a participant, an employee shall complete and deliver to the Personnel Officer the forms required by the Personnel Officer. Once an employee becomes an active participant, an employee may cease to be an active participant only as provided in this article.
