§ 3-2-104. Form of petition.
   (a)   Generally. In addition to the requirements of § 308(b) of the Charter, a petition shall be prepared in accordance with the provisions of this section.
   (b)   Signatures; required information. Each signature shall be followed by the residence of the signator, the election district and precinct where the signator is registered as a voter, and, immediately below the signature either printed or typed, the name of the signator. If the signator's residence differs from the residence recorded in the voter registration records, the signator shall state both residence addresses together with the date on which the current residence was assumed.
   (c)   Form of petition. Each sheet of paper forming part of a petition shall bear the text of the ordinance or part of the ordinance that is the subject of the petition.
   (d)   Signature on both sides of paper. Signatures may appear on both sides of a sheet of paper forming part of a petition. On the side of a sheet of paper that does not bear the text of the ordinance or the part of the ordinance there shall be a notice that the text of the ordinance or part of the ordinance that is the subject of the petition appears on the reverse side of the page. The notice shall be no smaller than eight- point bold type.
   (e)   Withdrawal of signature. A signator may not withdraw a signature after the petition has been filed in accordance with § 3-2-105.
