Each application for such permit shall be in writing, upon a form provided by the Plumbing Inspector for that purpose. It shall state the name and address of the applicant and a description of the property, whereon or wherein it is proposed to conduct a campground. It shall also contain such either information as the Plumbing Inspector may require, and it shall be filed by the applicant. It shall be filed with the Plumbing Inspector not less than five days nor more than thirty days before said campground is made ready for use, and it shall be accompanied by an inspection fee of one dollar and a fee of twenty-five cents per fixture, for which the Plumbing Inspector shall issue a receipt.
