(a) The fees for a sweepstakes terminal café license shall be:
(1) Ten thousand dollars ($10,000) per year, payable annually in advance; plus
(2) Fifty dollars ($50.00) per month for each computerized sweepstakes device at the sweepstakes terminal café, payable monthly in advance.
(b) The fee for a replacement license shall be fifteen dollars ($15.00).
(c) All fees required under this chapter shall be filed with the application for the license.
(d) In the event an application is denied under this chapter, the license fees, less two hundred dollars ($200.00) and less any costs incurred for reports and investigations, shall be returned to the applicant. No portion of the license or application fee shall be returned to the owner should the license be suspended or revoked.
