The Mayor is authorized and directed to confer with the Chairman of the District Advisory Council of the Lorain County General Health District and the Mayors of the cities of Oberlin, North Ridgeville, Avon, Sheffield Lake and Avon Lake in regard to the union of the health districts and to enter into a contract to combine the health districts and to form a combined board of health as provided for in Ohio R.C. 3709.07. Such contract shall provide:
(a) A combined board of health representing the cities of Oberlin, Amherst, North Ridgeville, Avon, Sheffield Lake, Avon Lake or any of these cities desiring to form a combined health district and representing the townships and villages of the County, and the terms of office of such board members;
(b) The effective date of such contract and the method of termination; and
(c) The proportion of expenses or the method of apportioning the expenses of the health districts that are forming the combined health district.
