A charitable or religious organization on whose behalf persons will engage in peddling, soliciting, or canvassing within the City shall furnish the following information to the Chief of Police:
(a) The name of the organization;
(b) The name, address and the telephone number of the person supervising the activity;
(c) The name of an officer or official of the organization;
(d) The nature of the charitable or religious purpose of the organization; and
(e) A list of the name(s) of persons who will be peddling, soliciting or canvassing.
