No person being the owner of a game room shall display for public patronage or keep for operation by the public, any gaming machines without first obtaining a license to do so issued by the City.
Applicants for a license under this chapter shall file with the City an application in writing on a form to be furnished by the Building Inspector which shall give the following information:
(a) Name and description of the applicant;
(b) Address (legal and local);
(c) A brief description of the nature of the business; and
(d) If the individual completing the application is an employer of the prospective licensee the application shall provide the name and address of the employer together with credentials establishing the exact relationship of the parties.
