All taxicab operators, taxicab drivers, personnel transportation operators and personnel transportation drivers prior to conducting any business within the corporate limits of the City of Alliance shall file an annual application with the Mayor’s office containing the following information:
(a) Name, address and phone number of the applicant;
(b) List of vehicles utilized in the business, including copies of titles or lease agreements, vehicle identification numbers and current proof of license registration;
(c) Names and addresses of drivers and operators;
(d) Name and address of insurance company and agent with certificate of insurance as required herein;
(e) Schedule of rates and fares;
(f) Pay of annual license fee of twenty-five dollars ($25.00) per motor vehicle utilized in the business;
(g) File a Certificate of Inspection by a mechanic for each motor vehicle utilized in the business advising that the motor vehicle is in good operating condition.
