745.03 APPLICATION.
   Every person desiring to open, conduct, operate or maintain what is commonly known as a public poolroom, billiard room or bowling alley shall make application to the Mayor for a license to do so. Such application shall be made upon a form to be furnished to the applicant and shall state the applicant's full name and address, including the street and number of his proposed place of business, age, date of final naturalization papers if foreign born and place of birth. If the applicant is a firm or corporation, such information shall be given with respect to the several members of the firm or the officers of the corporation. In the case of a society or association unincorporated, such information shall be given respecting the officer thereof.
   The application shall further set forth the name of the owner of the premises and his address, on what floor of such premises the business is to be conducted, whether the applicant has previously been engaged in such business, and when and whether such applicant has ever had a license suspended or revoked for the operation of such business. No license shall be granted to any person who is not a citizen of the United States or who has been convicted of a felony.
