The Director may suspend or revoke a license issued hereunder, after notice and hearing, on any of the following grounds:
(a) That the license was procured by fraudulent conduct, false statement, or nondisclosure of a material fact;
(b) That the licensee has violated a schedule of current base rates and hours filed with the Director or distributed by the licensee;
(c) That the licensee has violated any of the requirements of State law;
(d) That the licensee has violated any of the requirements of this chapter;
(e) That the licensee has violated any of the standards, rules or regulations promulgated by the Board of Health and pursuant to this chapter;
(f) For other good causes shown, and in this connection, a medical vehicle service, or other person engaged in the business of transporting sick and disabled persons in a nonemergency medical vehicle or vehicles, shall be responsible for the acts of its employees who are licensed as emergency medical technicians or as nonemergency medical vehicle operators.
