An applicant for a permit shall first file an application therefor in writing in the office of the Director of Community Development. The Director of Community Development shall prepare and furnish forms for that purpose. Each application shall state:
(A) The location of the job;
(B) The building or portion thereof to be sandblasted;
(C) The length of time it is estimated the job will take;
(D) The date when sandblasting work is to commence;
(E) The number of men to be employed; and
(F) The type of equipment which will be used.
