(A) No person shall construct or repair any sidewalk on any street, avenue or other public property in the city without first obtaining a permit; provided, however, that, no permit shall be required for any construction or repair ordered by or made by the city. There shall be no charge for a sidewalk permit.
(B) All applications for permits required by division (A) above shall be made on forms approved by the Council. All applications shall state:
(1) The date construction or repair will begin; and
(2) The time for completion, and sufficiently describe the contemplated construction or repair.
