In addition to the application requirements specified in Section 5.04.100, the following shall be required:
A. A statement that the applicant, licensee, his agent, employee or any person connected with the licensee as partner, director, officer, stockholder, general manager or other associate has not:
1. Knowingly made any false, misleading or fraudulent statement of a fact in an application for a license or in any statements made to City departments, the local fire authority or the Police Department investigating the application; or
2. Violated any provision of this Chapter or any statute relating to his permitted activity; or
3. Been convicted of a felony or any crime involving moral turpitude; or
4. Committed any unlawful, fraudulent or dangerous act while conducting the permitted business; or
5. Violated any rule or regulation adopted by the City Council relating to the licensee’s business; or
6. Conducted the licensed business in a manner contrary to the peace, health, safety or general welfare of the public; or
7. Failed to comply with zoning, building and safety regulations of the City, the San Bernardino County Environmental Health Services Department, or the Adelanto fire district prior to the commencement of business operations.
