5.04.100   Applications.
Every person required to have a business license shall make written application to the City Clerk, The applicant shall furnish all information required to enable the City Clerk to properly classify the business of the applicant and determine the proper license tax or fee to be paid by such applicant.
The business license application shall be provided by the City Clerk, and shall require, but not be limited to, the following information:
   A.   The exact nature or kind of business for which the license is required.
   B.   The address where such business is to be carried on or conducted.
   C.   The name, address, driver’s license number, phone number and mailing address of the owner of the business and any fictitious business names used by the owner.
   D.   In the event that the application is made for the issuance of a license to a corporation or partnership, the application shall set forth the names and places of residences of the officers or partners thereof.
   E.   An affirmation by the applicant that the business does not furnish or provide any service, good, or product that is illegal under this Code, the laws of the State of California or the laws of the United States of America.
   F.   The application shall be signed by the applicant or duly authorized agent of the applicant.
   G.   In cases where such business is not carried on or engaged in a permanent place of business in the City, that fact shall also be stated in the application and in addition to the address of the person engaging in or carrying on such business shall be stated thereon.
   H.   No license shall be issued to any person conducting or carrying on any business under a fictitious name, unless an affidavit is filed in the office of the County Clerk or in the office of the City Clerk, showing the true names of the owners of such business; provided, that such license may be issued in the true names of the owners of such business without the filing of such an affidavit.
   I.   No license shall be issued to any person conducting or carrying on any business until all requested documentation is provided and the payment of the license fee as herein requested.
   J.   No license shall be issued to any person conducting or carrying on any business which is illegal under the laws of the State of California or under the laws of the United States of America.
