A. 1. Any person desiring to erect any new building, to remodel any existing building, to repair by replacement with new materials different from existing worn materials, to add on, or to alter or change the structure of any existing building, shall first obtain a building permit application form from the office of the city clerk.
2. Any person desiring to repair by replacement, including overlay, with new materials different from existing worn materials, to any existing roof of a residential home or building shall first obtain a building permit application form from the office of the city clerk. A permit fee will be assessed by the clerk of twenty dollars ($20.00) for the issuance of a building permit for and such to repair by replacement. (Ord. 2016-8, 4-18-2016)
B. When obtaining a building permit application form from the city clerk, the person shall file in writing with the clerk a statement of his intention as to the building or buildings involved, which shall become part of his application for said permit. (2006 Code § 4-1-2)
C. The city clerk shall furnish the form to be used for such applications, and the use of said form in making the application shall be required. (2006 Code § 4-1-3)
