Application for an excavation permit shall be made to the city clerk on a form prescribed by the corporate authorities and prepared by the city clerk and shall be accompanied by payment to the clerk of a fee of ten dollars ($10.00) for the filing of each application for an excavation permit, which fee is not returnable in the event that the permit is denied.
