Every person desiring to do any excavation work shall apply to the town administrator for a permit therefor on a form provided by the town, stating the applicant's name, the location, length, width and purpose of the proposed excavation, the amount of the permit fee, the dates of commencement and completion of the work and a statement that the work will be performed in strict compliance with the plans, specifications, and procedures furnished by the town administrator.
