(a) The application for a parade permit shall be filed at least five days prior to the date of the proposed parade with the Village Police Chief or the Village Police Chief's designee.
(b) Applications shall be upon a form furnished by the Village. Each application form shall require full, complete and detailed information, including the following:
(1) The name, address and telephone number of the person(s), entity or organization seeking to conduct the parade;
(2) The name, address and telephone number of the headquarters of the organization, if any, and of the organizer or responsible head of such organization by whom or on whose behalf the parade is proposed to be conducted;
(3) The name, address, and telephone number of the person who will chair the parade and who will be responsible for its conduct;
(4) The street or other public property and the specific area or areas which will be utilized in connection with the proposed parade, including the location of the proposed assembly area and disbanding area;
(5) The approximate number of persons who will be participating in the proposed parade;
(6) The hours when the parade will start and conclude;
(7) The type of security arrangements that will be provided;
(8) A statement to be signed by the person seeking the permit that the permittee will hold harmless and indemnify the Village, its elected officials, officers, employees and agents from any damages which may arise as a result of the conduct of the parade or assembly for which the permit is sought by persons who were or reasonably should have been under the control of the permittee.
