§ 810.02 LICENSE APPLICATION.
   The applicant for a license shall file an application in writing with the City Manager showing the location and type of structures proposed to be used. The application shall contain the following:
   (a)   The name of the applicant;
   (b)   The location of the principal place of business of the applicant;
   (c)   The names and addresses of the officers if the applicant is a corporation;
   (d)   The effective period of the license applied for;
   (e)   The proposed connections to the municipal water system and sanitary sewerage system, including provisions for public and private toilet and sanitary facilities;
   (f)   The type of construction and equipment;
   (g)   The location of electrical service; and
   (h)   The location of exits.
