All persons testifying at any hearing pursuant to the rules set forth in this chapter shall stand and be administered the following oath by either the presiding officer or secretary of the agency:
Do you swear (or affirm) to tell the truth, the whole truth and nothing but the truth in this hearing now before the (name of agency)?
