(A) The driver of any vehicle for hire, as well as the owner thereof, shall be responsible for insuring that the following items are displayed so as to be visible to passengers:
(1) The vehicle for hire license shall be affixed to the upper left hand corner of the driver’s side front window of the vehicle;
(2) A photograph of the driver;
(3) A schedule of fares.
(B) The driver of a vehicle for hire shall, upon the request of any passenger, give to such passenger the correct name and address of such driver and such other information as will identify the vehicle, driver and owner thereof.
