Every licensee shall provide upon request a receipt to the seller or consignor of any item described in § 112.07 which shall include:
(A) The name, address, and phone number of the licensed business;
(B) The date on which the item(s) was received by the licensee;
(C) A description of the item(s) pawned or purchased;
(D) The amount paid for the items and each of them;
(E) The signature of the licensee or its agent;
(F) If pawned, a date after which the item(s) will be sold or otherwise disposed of by the licensee.
