139.16 DISCIPLINE.
   The primary responsibility for the administration of discipline shall rest upon the immediate supervisor of the employee to be disciplined, although if he fails to take action, which in the opinion of his superior is justified, such superior may himself impose the discipline. Disciplinary action taken against an employee, which is other than in the nature of a minor first offense warning shall be in writing and made a part of such employee's permanent personnel file. The employee shall be given notice of any disciplinary action taken against him together with the reasons therefor not later than the effective date of such action. The notice shall inform the employee of his rights to have his case reviewed in accordance with the provisions of Section 139.17, supra.
