(A) Each individual proposing to canvass, peddle, or solicit from door to door for the purpose of soliciting money for any cause, or taking orders for merchandise, or advertising, except as hereinafter listed, shall apply for a license with the City Clerk. Such application shall contain the following:
(1) The name of the organization applying for the license to solicit, canvass, or peddle and the address of its headquarters;
(2) The names and addresses of its principal officers and management;
(3) The purpose for which such solicitation, peddling, or canvassing is to be made;
(4) The name and address of the person or persons who will be making the solicitation, peddling, or canvassing;
(5) The time when such solicitations, peddling, and canvassing shall be made, giving the preferred dates for the beginning and ending of such solicitation;
(6) Such other information as may be required by the City Manager and City Council, in order that they may fully determine the kind and character of the proposed solicitation, and as to whether such solicitation is in the interests of, and not inimical to the public welfare;
(7) A statement to the effect that if a permit is granted it will not be used or represented in any way as an endorsement by the City Manager and City Council, or by any member thereof, or by the city; and
(8) A certification from the Police Department, signed by the Police Chief, that a record check has been made of the applicant or applicants, and that approval or disapproval of the license is recommended.
(B) Said application shall be accompanied by the payment of $25 for each person named in division (A)(4) above, and an additional $25 for each day of intended solicitation as set out in division (A)(5) above.
