(A) The application for such a license shall contain the following information:
(1) Name and address of the applicant, and applicant’s age, date, and place of birth;
(2) All prior convictions of felonies of the applicant, if any;
(3) Address and name of business where the machine or device will be displayed and operated and the nature of the business conducted at that address and under that name; and
(4) The name and address of the owner of said machine and if said machine is serviced and supplied by any person, firm, corporation, or association other than the applicant or the owner of said machine, the name and address of such person, firm, or association shall be set out in said application.
(B) No license shall be issued to any applicant unless he or she shall be over 21 years of age and a citizen of the United States.
