§ 96.31 PROCEDURE.
   The Director of Public Services is hereby authorized to prepare a permit application and to supervise the issuance of such a permit according to the terms of this subchapter. As a condition precedent to the issuance of said permit, the applicant shall pay to the city the amount of $250. With regard to all streets, oil and chip, concrete, and asphalt street cuts, the contractor will be solely responsible for the repair. The repair shall be done in such a manner that is described on the street cut permit form, or to the satisfaction of the Public Services Director or his or her designee. Upon completion, $150 will be refunded. Further, in consideration of being granted a permit, the licensee must agree to indemnify and hold forever harmless, the city, its agents, and servants against and from any and all claims and cause of actions which may arise or which may be brought by anyone, for any kind of damage, or personal injury which might arise from the excavation permitted by granting of such permit.
