(A) Before any license shall be issued as provided herein, the applicant shall submit his or her written application therefor, upon forms provided by the city, to the City Manager, which application shall disclose:
(1) The name and address of the applicant;
(2) The park or playground for which the license is sought;
(3) The types of food and drink to be sold;
(4) The purpose to which the proceeds of such sales shall be applied; and
(5) The months of the year, the days of each week, and the hours of each day when such sales shall ordinarily be made.
(B) The application shall be accompanied by the following annual fees:
(1) The annual fee for a nonprofit civic organization shall be $1; and
(2) The annual fee for an individual, firm, or corporation operated for profit shall be $25.
