§ 113.11 APPLICATION; FEES.
   (A)   The application for a license shall contain the name and address of the applicant, the type of business proposed to be conducted or maintained, and the address of the place where it is proposed to conduct such profession, occupation, or business. ('79 Code, § 1513(3))
   (B)   Upon receipt of application for the license, the health inspector shall make a complete and thorough examination of the premises wherein the applicant proposes to operate the business to determine if the premises comply with the provisions of this section. The health inspector shall determine to his satisfaction that such person is in sound physical, mental, and moral health and of normal intelligence, a graduate of an accredited institute or school, and as being possessed of sufficient knowledge of the human body and its anatomy to be able to practice thereon without danger of inflicting bodily injury or other harm through ignorance, inability, or other personal defects. The Village Clerk shall require from the applicant references showing the previous places of employment, if any, and upon being satisfied that the applicant has the requisite qualifications to engage in such business or profession may grant a license to such applicant. The provisions of this section shall not apply to owners, lessees, or employees who themselves do not practice on the human body.

   (C)   The annual fee for the license shall be $300 for each establishment, and in addition $50 for each operator, renewable yearly.
