§ 151.32 PERMIT APPLICATION.
   No building, structure, or other location improvement permit shall be issued by the Administrator unless the application for a permit is accompanied by:
   (A)   A plat or sketch of the proposed site, including a legal description, depiction of all lot corners, and location of all existing and proposed improvements;
   (B)   Plans and specifications, showing the work to be done;
   (C)   A drainage plan showing the location of any legal drains, and of any existing or proposed subterranean or surface drainage systems for stormwater storage, detention, retention, or discharge. The plan must also indicate the direction of flow of all existing or proposed overland storm flow;
   (D)   Existing and proposed elevations, which shall be referenced to M.S.L. datum, at:
      (1)   All lot corners;
      (2)   All exterior grades and finished floors of any buildings;
      (3)   Any driveways;
      (4)   Any driveway culverts or bridges;
      (5)   Any public streets, curbs, and storm drains on or adjoining the site;
      (6)   Any ponds, lakes, or streams on or adjoining the site;
      (7)   Any drainage structures shown in division (C) above; and
      (8)   Of any proposed footings.
   (E)   Proof of bonding or other surety approved by the town, for any improvements to be made on public property or within any public right-of-way, if deemed appropriate by the Administrator, Town Engineer, or the town;
   (F)   A copy of the design release, issued by the State Building Commissioner and State Fire Marshal pursuant to I.C. 22-15-3-1, shall be provided for construction covered by the design release;
   (G)   Exceptions to divisions (C) and (D) above, or any portions thereof, may be granted at the discretion of the Administrator and/or the Town Engineer, if the type of location improvement permit does not significantly affect stormwater drainage and flow at or near the project site; and
   (H)   Any other documents, as required by § 156.372, or by the Administrator or the Town Engineer.
   (I)   A non-refundable, administrative application fee to cover the cost of processing the building permit application. The fee shall be in the amount of:
      (1)   $50 per permit (residential); or
      (2)   $400 per permit (commercial).
