§ 111.19 INSURANCE REQUIRED.
   Each contractor must provide, on an approved form, a certificate of insurance, issued with his or her business entity as the insured, by an insurance provider licensed in the state, rated B+ or better. The certificate must show proof of general liability for property damage and personal injury in an amount not less than $500,000 for each occurrence, or annual umbrella liability coverage in an amount not less than $1,000,000. Contractors with employees must also show proof of worker’s compensation and employer’s liability insurance in amounts required by the state. Should the contractor’s required insurance lapse prior to expiration of the registration, the contractor’s registration will immediately and automatically be placed on inactive status. It is the responsibility of the contractor to provide a current certificate of insurance to the Administrator or staff if he or she desires to maintain an active status registration.
