(A) No person shall operate or maintain a retail, services, or other business establishment within the town without first obtaining a business license therefore as provided in this chapter.
(B) An applicant for a business license issued under this chapter shall be made upon a blank form prepared and made available by the Clerk-Treasurer and shall state the following information:
(1) The name, home address, phone number, and existing or proposed business address of the applicant;
(2) The name, address, and phone number of the owner of the real estate upon which the business is located;
(3) The hours of operation for the proposed retail and service establishment; and
(4) Any other information the town shall find reasonably necessary to effectuate the purpose of this chapter and to arrive at a fair determination of whether the terms of this chapter have been complied with by the applicant.
