(A) Every applicant for a license under this section shall furnish the Clerk/Treasurer with:
(1) The name and address of the operator and manager of the equipment or premises to be licensed;
(2) The name and address of the owner of the equipment or premises to be licensed; and
(3) A licensing fee equal to $15 for one table, alley, machine, device, or game for hire or pay and $10 for each additional table, alley, machine, device, or game for hire or pay.
(B) (1) The Clerk/Treasurer shall not issue any license under this section until the applicant has complied with division (A) above and the application has been approved by the Council.
(2) The Council may issue a license subject to the compliance of the license holder with such restrictions on the hours and manner of operation as the Council deems proper.
