The plan covers all full-time employees of the city, subject to certain exclusions. Plan eligibility, benefits, exclusions, deductions, and other provisions relevant to the plan are set forth in the "Summary Plan Description" annexed hereto, the provisions and conditions of which are incorporated herein and made a part of this subchapter by reference. Two copies of the Summary Plan Description are available for public inspection at the office of the Clerk-Treasurer.
