755.04 PERMIT APPLICATION FEE.
   (a)   Prior to operating a massage establishment within the City of Willoughby Hills, every operation shall obtain a permit to operate from the City. The permit to operate shall expire on December 31 following the date of issuance and shall be renewed annually in the month of December and before the first day of January of the next succeeding year.
   (b)   An annual permit fee of one hundred dollars ($100.00) shall be paid by each massage establishment operating within the City of Willoughby Hills. An application for a permit to operate a massage establishment shall be accompanied by a fee in the amount of one hundred dollars ($100.00). Any change of ownership shall require an application and permit with payment of associated fees.
   (c)   It shall be unlawful for any person, firm or corporation to engage in the business of operating a massage establishment within the City of Willoughby Hills without a permit issued by the City to engage in such business. Such a permit shall be displayed in a conspicuous place within the establishment.
   (d)   The failure or refusal of the applicant to promptly give the information relevant to the investigation of the applicant or his or her refusal to submit to or cooperate with any inspection required by this Chapter shall constitute an admission by the applicant that he or she is ineligible for such permit and shall be grounds for the denial thereof by the City.
