749.03 REQUIREMENTS FOR LICENSE.
   In order for any person to obtain the license required in Section 749.02, that person shall have filed and approved with the office of the Director of Public Safety in such form as required by the City, the following:
   (a)    A plan or diagram drawn to scale showing the available parking arrangements to accommodate the proposed number of persons attending which diagram shall have been submitted to and approved by the Fire Chief. The parking to be provided shall be based on a ratio of three occupants per vehicle. Any designated parking area must be maintained in a usable and accessible condition. In determining the sufficiency of proposed parking no consideration shall be given to nearby parking required by law for permanent commercial structures at or near the site for which license application is made. The proposed parking shall be located on, or on property contiguous to, the proposed picnic site and shall not be separated from such site by any dedicated public street or highway.
   (b)    A plan or diagram drawn to scale stating the total approved number of persons attending such event with a plan as to the evacuation of such event in case of a fire or other emergency; such plan to be submitted to and approved by the Fire Chief.
   (c)    A statement as to crowd control and security in order to prevent damage or destruction of any neighbor's property and a plan of traffic control. The plans to be submitted to and approved by the Chief of Police.
   (d)    The results of a noise level test conducted at or near to the subject location on the site of such event which test shall certify the average audible level of noise, expressed in decibels, that such site experiences without such event being in process. A statement, approved by the Chief of Police as to noise level control so that the audible level of noise shall not exceed 1.5 times the average audible level as determined pursuant to this subsection.
   (e)    A statement of the hours that the event shall be open but in no case shall the event commence prior to 10 a.m. or continue past 11 p.m.
   (f)    A current certificate of testing of all equipment, installations and wiring issued by the City Fire Department and Electrical Inspector; such certificate to demonstrate that within the last six months such equipment, installations and wiring has been inspected and tested for safety by such safety inspectors.
   (g)    An accord certificate of insurance signed by an insurance company licensed to do business in the state of Ohio which shows that coverage is in full force and effect in limits not less than five hundred thousand dollars ($500,000); except that where the event shall contain any rides or form of transportation, the limits shall be not less than two million dollars ($2,000,000); with coverage for all the following on a comprehensive form:
      (1)    Premises liability;
      (2)    Products completed operations liability;
      (3)    Liquor liability insurance, if appropriate, for the sale and distribution of all alcoholic beverages;
      (4)    Motor vehicle liability insurance;
      (5)    Blanket contractual liability insurance;
      (6)    Personal injury liability;
      (7)    Employers liability insurance;
      (8)    With certification that the City shall have thirty days advance notice of cancellation, nonrenewal or change in the coverages or limits contained in such policy or policies.
   (h)    No license shall be issued to any applicant who has violated the terms and conditions of a prior license issued under the authority of this chapter within thirty days of such (additional) application.
