(a) Application for a license to display a mechanical amusement device shall be made to the Director of Public Safety upon such forms as prepared by the Director of Public Safety. The application shall be made by the owner or proprietor of the business or place at which the mechanical amusement device is to be displayed. The application shall state the name of the owner of the place or business, the address of the place at which the mechanical device is to be displayed, the residence address of the owner, the serial number and name of manufacturer of the mechanical amusement device, the owner of the mechanical amusement device, the name and address of the distributor of the mechanical amusement device, the nature of the business in conjunction with which the mechanical amusement device is to be displayed and such other information as may be required by the Director of Public Safety.
(b) No application for an amusement device license shall be granted and no amusement device license shall be issued until the applicant has furnished to the Director of Public Safety, evidence of public liability insurance for bodily injury in the minimum amount of one hundred thousand/three hundred thousand dollars ($100,000/$300,000) and for property damage in the minimum amount of fifty thousand dollars ($50,000).
