An application for a license under this chapter shall be submitted to the Chief of Police on the form supplied by the Bureau of Codes setting forth the following:
(a) The name and address of the applicant;
(b) Premises from which licensed business, operation or activity is to be conducted;
(c) Nature of proposed business, operation or activity;
(d) Any previous criminal record of applicant, applicant’s employees and applicant’s employer, principal or agent;
(e) If applicable, names and address of person(s) by whom applicant is employed, organization on whose behalf applicant is engaging in business, operation or activity or principals or agents of applicant; and
(f) Names and address of employees.
