§ 154.040 SITE PLAN; FINAL AND PRELIMINARY.
   (A)   (1)   Information required at the time of application (one copy of each):
         (a)   Completed application and fees; and
         (b)   Proof of ownership (title policy) and owner’s written authorization.
      (2)   Information required directly on plans (25 copies of each to be submitted with application - sheet size shall be no less than eight and one-half inches by 11 inches and no more than 30 inches by 36 inches):
         (a)   Date, north arrow and engineer’s scale, one inch equals 200 feet or larger;
         (b)   Name of project;
         (c)   Property address or location;
         (d)   Legal description complete;
         (e)   Registered surveyor’s name and address;
         (f)   Total acreage;
         (g)   Signature blocks (see § 154.072 of this chapter);
         (h)   Zoning on and adjacent to site;
         (i)   Names and dimensions of adjacent streets;
         (j)   Dimensions and square footage of each lot;
         (k)   Lot and block numbers;
         (l)   Proposed street names;
         (m)   Bearing, distances, chords, radii, central angles, tangent lines and the like for all lots, blocks, perimeter and rights-of-way;
         (n)   Existing/proposed adjacent street improvements showing pavement widths and intersecting streets;
         (o)   Existing/proposed rights-of-way in and adjacent;
         (p)   Location and widths of sidewalks;
         (q)   Existing/proposed easements in and adjacent;
         (r)   Existing/proposed utility lines (including fire hydrants) and sizes in and adjacent;
         (s)   Existing/proposed curb cuts in and adjacent;
         (t)   Existing/proposed waterways and ditches in and adjacent;
         (u)   Existing/proposed floodplain lines on and adjacent;
         (v)   A statement: all, part or none of the property is in the floodplain;
         (w)   Existing/proposed structures and paved areas on the site;
         (x)   Proposed heights of all structures;
         (y)   All public sites to be reserved or dedicated (parks, streets and the like);
         (z)   Landscape plan;
         (aa)   Percentage of land devoted to buildings, parking and drives, sidewalks and landscaping in table form;
         (bb)   Number of employees for non-residential uses and required parking spaces; and
         (cc)   Latitude, longitude and elevation on property corners.
      (3)   Information required for review-to be submitted with application (one copy of each, except as noted):
         (a)   Boundary closure;
         (b)   Letter from County Treasurer verifying payment of taxes; and
         (c)   Soil report sealed by a registered engineer (three copies).
      (4)   Additional information required prior to recording (one copy of each to be submitted to Planning and Zoning Official after final City Council approval):
         (a)   Final street plans and profiles stamped by a registered engineer;
         (b)   Final drainage plan stamped by a registered engineer;
         (c)   Final utility plan stamped by a registered engineer;
         (d)   Final landscaping plan; and
         (e)   Financial security agreements.

         (t)   Statement on how drainage will be handled;
         (u)   Existing/proposed floodplain lines on and adjacent;
         (v)   A statement: all, part or none of the property is in the floodplain;
         (w)   Existing/proposed structures and paved areas on the site;
         (x)   Proposed heights of all structures;
         (y)   All public sites to be reserved or dedicated (parks, streets and the like);
         (z)   Landscape plan;
         (aa)   Percentage of land devoted to buildings, parking and drives, sidewalks and landscaping in table form;
         (bb)   Number of employees for non-residential uses and required parking spaces;
         (cc)   Proposed use of site;
         (dd)   Gross and net residential densities of each parcel;
         (ee)   Latitude, longitude and elevation of property corners; and
         (ff)   Phasing plan.
      (3)   Information required for review-to be submitted with application (one copy of each, except as noted):
         (a)   Capacity and need for water and for sewer;
         (b)   Stamped, addressed envelopes for all property owners within 300 feet of proposed development;
         (c)   Construction schedule;
         (d)   Protective covenants if proposed;
         (e)   Traffic impact analysis (three copies);
         (f)   Drainage report (three copies); and
         (g)   Water agreement (three copies).
