§ 74.19 ISSUANCE OF CITATION.
   (A)   Any Deputy Sheriff of this county shall conspicuously attach to any vehicle committing or having committed a parking violation a citation on a form as described in division (B) below, or shall hand said citation directly to the owner or driver if he or she is present when the citation is written. Such citation shall advise the owner or operator of the vehicle that he has violated a provision relating to the parking provisions of this chapter.
   (B)   The form of said citation shall contain space to record the make and model of the violator's vehicle and the vehicle license plate number. The citation shall also contain the categories of violations, which shall be:
      (1)   General parking violations (as found in §§ 74.02 through 74.10). The officer shall indicate which violation is applicable to the offending vehicle. The fine shall be $35.00, if paid within 14 days of issuance of the citation.
      (2)   Prohibited parking, handicapped persons (as found in § 74.11). There shall be a $250 fine, in addition to any costs or charges for removal of the vehicle, if paid within 14 days of the date of issuance of the citation.
      (3)   Violations (as found in §§ 74.14 and 74.15). The officer shall indicate which violation is applicable to the violator. The fine shall be $50, if paid within 14 days of issuance of the citation.
   (C)   The citation shall also contain directions as to payment of the fine within 14 days of issuance of the citation, allowing for payment by mail or at the Will County Sheriffs Department Office, and shall specify the fine schedule for the offense charged. The citation shall conspicuously inform the violator that if the fine is not paid within 14 days of issuance of the citation that the violation shall be referred to the Administrative Adjudication process. If the person who has received the citation wishes to contest the charges, then they shall file the violation notice with the “Request for Hearing” portion fully completed with the Traffic Compliance Administrator postmarked within ten days of the violation notice issuance. The request shall be deemed filed upon receipt by the Traffic Compliance Administrator.
   (D)   The form shall be signed, either handwritten or electronic signature or identification number, by the officer issuing the citation.
